INTRODUCTION

t o th e s o c i o - c u l t u r a l f a b r i c and assume f u l l s i g n i f i c a n c e
o n ly when p e r c e i v e d a s p a r t of th e t o t a l d e s i g n . An im
p o r t a n t a i d t o u n d e r s t a n d i n g th e community and i t s r e
a c t i o n t o a m e d ic a l o r h e a l t h problem i s th e c o n c e p t of
c u l t u r e w ith i t s a c c e n t on p a t t e r n and f u n c t i o n as w e l l
a s on th e s p e c i f i c ite m s of b e l i e f and p r a c t i s e . " ^ The

Man can n o t be ta k e n out of h i s e n v iro n m en t and
c o n s id e r e d a s a t h i n g a p a r t . Man i s s u b j e c t t o c e r t a i n
s o c i a l la w s, and i s p a r t o f a s o c i a l s t r u c t u r e . I f he i s
removed from t h i s s o c i a l e n v iro n m e n t, th e n o n ly a p a r t i a l
and in c o m p le te u n d e r s t a n d i n g i s p o s s i b l e . V a rio u s e le m e n ts
of t h i s s o c i a l e n v iro n m e n t have im p o rta n t b e a r i n g s on th e
b e h a v i o r and a t t i t u d e s of man. These b e h a v i o r s and a t t i
tu d e s a r e i n t i m a t e l y r e l a t e d t o m an’s t o t a l l i v i n g , h i s

The p a t h o l o g i c a l p r o c e s s of t u b e r c u l o s i s i s c a u se d
by th e p re s e n c e i n th e body o f th e t u b e r c l e b a c i l l u s o rl
M ycobacterium t u b e r c u l o s i s . While t h e r e a r e many t y p e s of
t u b e r c l e b a c i l l u s , o n ly two of t h e s e , t h e human and b o v in e
v a r i e t i e s , a r e o f p r a c t i c a l im p o rta n c e a s t h e c au se of
d i s e a s e i n man. The human b a c i l l u s i s r e s p o n s i b l e f o r th e
v a s t m a j o r i t y of c a s e s of t u b e r c u l o s i s . A p p a r e n tly , t h e r e
i s no d i f f e r e n c e i n th e form s of d i s e a s e c a u se d by th e
human and M. b o v i s t y p e s , n e i t h e r i s t h e r e any c o n c lu s iv e
e v id e n c e of d i s s i m i l a r i t y i n th e d e g re e of v i r u l e n c e . I f
t h e r e i s a d i f f e r e n c e i n th e v i r u l e n c e , i t h a s n o t demon
s t r a t e d any p a r t i n d e te r m in in g th e c l i n i c a l c o u rs e of
d i s e a s e i n man.

c u l o s i s . The n a t u r e of r e s i s t a n c e t o th e i n f e c t i o n i s
n a t i v e o r a c q u i r e d . I n e i t h e r c a s e , th e developm ent of

t u b e r c l e b a c i l l u s . I t i s t h e r e f o r e h i g h l y d e s i r a b l e to
s t r i v e t o g a i n more i n f o r m a t i o n a s t o th e e le m e n ts which
a r e b e l i e v e d t o g r e a t l y i n f l u e n c e t u b e r c u l a r i n f e c t i o n
w h e re v e r t h e d i s e a s e i s fo u n d . F o r t h i s p a p e r , th e w r i t e r

NATIONAL PROBLEM
H i s t o r y
V i t a l s t a t i s t i c s w hich i n c l u d e d d e a t h from t u b e r
c u l o s i s h a s b e e n a v a i l a b l e i n Jap an s i n c e 1 89 9 . A few
y e a r s e a r l i e r , 1 8 9 3 , New York C i t y was t h e f i r s t g e o
g r a p h i c a l a r e a i n t h e U n ite d S t a t e s t o r e q u i r e t h a t a l l
c a s e s of t u b e r c u l o s i s i n p u b l i c i n s t i t u t i o n s and a l l c a s e s
u n d e r t h e c a r e of p r i v a t e p h y s i c i a n s (1897) sh o u ld be r e -
p o r t e d t o t h e H e a l t h D e p a rtm e n t.
A f t e r t h e R e s t o r a t i o n i n J a p a n , th e I m p e r i a l g o v
ernm ent u r g e d p r o p h y l a c t i c t r e a t m e n t a g a i n s t t u b e r c u l o s i s .
In 1890 when K och’ s t u b e r c u l i n was f i r s t p r e p a r e d , t h e
J a p a n e s e M i n i s t r y o f Home A f f a i r s i s s u e d a d e c r e e con
c e r n i n g t h e u s e of K och’s t u b e r c u l i n and b y 1904 th e same
d e p a rtm e n t i s s u e d an o r d in a n c e r e l a t i n g to p r e v e n t i o n o f
p u lm o n a ry t u b e r c u l o s i s . ^ The d e c r e e s t a t e d t h a t i n a l l

The f i r s t t u b e r c u l o s i s s a n a to r iu m was b u i l t i n
Germany i n 1853 and D r. T ru d eau b u i l t t h e f i r s t s a n a
to r iu m f o r t u b e r c u l o u s p a t i e n t s i n th e U n ite d S t a t e s i n
t h e s t a t e of Hew York i n 1 88 4 . I n Ja p a n t h e go vern m en t
e n a c t e d a law i n March 1914 f o r th e e s t a b l i s h m e n t of
s a n a t o r i a i n c i t i e s w i t h 500 ,000 p e o p le o r m ore. F iv e
y e a r s l a t e r i n 1919, t h e "Law f o r t h e P r e v e n t i o n of
T u b e r c u l o s i s " p r o v i d e d f o r s a n a t o r i a i n c i t i e s of n o t
l e s s t h a n 5 0 ,0 0 0 p e o p l e .
A cco rd in g t o t h e above d a t a , J a p a n had some
i n f o r m a t i o n a b o u t t u b e r c u l o s i s v e ry e a r l y i n th e h i s t o r y
of t h e c o u n t r y . Cam paigning a g a i n s t t h e s p r e a d o f th e
d i s e a s e b e g a n i n J a p a n , Germany, and t h e U n i t e d S t a t e s
a t a p p r o x i m a t e ly t h e same t i m e . As f o r t h e m o r t a l i t y

The J a p a n e se W hite C ross S o c i e t y was o r g a n iz e d i n

g iv e n to c h i l d r e n was a s p e c i a l a tte m p t t o d ia g n o s e t u
b e r c u l o s i s a t th e tim e when t h e d i s e a s e t a k e s such a h ig h
t o l l . The J a p a n e s e W hite C ross S o c i e t y was th e f i r s t in
Jap an t o s t a r t a p erm an en t open a i r s c h o o l f o r c h i l d r e n .
In 1913 th e Ja p a n e s e A s s o c ia t io n f o r th e P r e
v e n t i o n of T u b e r c u lo s is was o r g a n iz e d . T h is o r g a n i z a t i o n
fo u n d ed c e n t e r s i n t h e p r o v i n c e s f o r t h e p u rp o se of p r o
p a g a t in g e d u c a t i o n a l i n f o r m a ti o n t o t h e p u b l i c c o n c e rn in g
p r e v e n t i o n of t u b e r c u l o s i s . I t i s s t i l l i n e x i s t e n c e and
h a s s e n t t o t h e w r i t e r much of th e m a t e r i a l w hich a p p e a rs

A f t e r World War I e c o n o m is ts and d em o g rap h ers to o k

They n o te d t h a t d u r i n g th e war p e r i o d th e t u b e r c u l o s i s
d e a t h r a t e had r e a c h e d a l e v e l e x c e e d in g t h a t o f any o t h e r
i n d u s t r i a l i z e d c o u n tr y i n t h e w o rld . I n 1919 th e y p a s s e d
t h e T u b e r c u l o s i s P r e v e n t i o n Law. However, I t was t h e
e a r th q u a k e of 1923 w hich awakened Jap an t o an a c t i v e
a w a re n e s s of th e n a t i o n ’ s n e e d s .
The e a r th q u a k e o f 1923 w ith i t s t r a i n o f d e s t i
t u t i o n on a l a r g e s c a l e gave a s t r i k i n g im p e tu s t o
e f f o r t s to m eet n o t o n ly th e a c u te b u t a l s o th e
c h r o n ic n e e d s of J a p a n . B e fo re t h a t d a te t h e r e had
b e e n l e g i s l a t i o n f o r t h e im provem ent of i n s a n i t a r y
h o u s in g , f o r th e p r e v e n t i o n of t u b e r c u l o s i s and f o r
employment a g e n c i e s . W orld War I had a l s o s t i m u l a t e d
e f f o r t s f o r p o o r r e l i e f , f r e e m e d ic a l a t t e n t i o n ,

d e a t h r a t e showed no d e c r e a s e b u t a te n d e n c y t o i n c r e a s e
i n r e l a t i o n t o p e r c e n t a g e of t h e e n t i r e number of p a
t i e n t s . No r e a s o n was g iv e n f o r th e c h a n g e s in t h e r a t e s .
R e g a r d le s s of t h e a r e a , i t i s u n u s u a l f o r th e
u p p e r c l a s s t o have th e h i g h e s t t u b e r c u l o s i s in c i d e n c e or
m o r t a l i t y r a t e . The i n v e s t i g a t o r s m e n tio n e d t h a t th e s u s
p e c t e d s o u rc e of i n f e c t i o n was n o t i n t h e v i l l a g e s . The
