FOREST LAWN MEMORIAL-PARK ASSOCIATION agrees that the balance outstanding up to $1,000 under
Application and Agreement for thejnirchase rjf interment progerty executed on this date by_

as purchaser
laser, other than by his own hand or act while sane or
insane, and interment in Forest Lawn Memorial-Park; provided that no payment under said contract is more than 31
days overdue at date of death, that said purchaser on date hereof is in good health and not over 60 years of age as
represented below, and that if purchaser has any other Guaranteed Ownership Plan with Forest Lawn Memorial-
Park Association, the total amount credited on all Applications and Agreements under Guaranteed Ownership
Plan shall not exceed $1,000.00. This provision shall apply only upon the death and interment in Forest Lawn
of the above named original purchaser, and shall be effective upon receipt of the first payment provided for by
said contract. Premiums for said insurance shall be paid by Forest Lawn Memorial-Park Association.
